As the school year comes to an end, the last thing most parents are thinking about is the “back to school” to-do list. But here at Weiss Pediatric Care, that’s exactly what we’re doing.

We know that many of you will be looking for health and immunization forms that are required for students transferring to new schools, entering kindergarten, 7th grade, or attending daycare.  Many sports teams require a sports health form prior to participation, too.

So as the summer heats up, so do requests for check-ups and forms.

Here are a few tips to help ensure that you have everything you’ll need when the school bells ring again in the fall:

  • If your child had a well visit within the past year, we have good news for you. Call, email, or message us through your child’s portal and your child’s health and immunization forms will be waiting for you to view and print in your child’s portal within 72 hours. If you’d prefer to pick up these forms, just let us know and we’ll have them ready for you within 72 hours.
  • If your child has not had a well visit in the past year, please call to schedule an appointment as soon as possible. The summer schedule is already very busy so waiting until the last minute makes it difficult to meet important school and sports deadlines.
  • Sports Forms typically can be completed based on a well visit within the past 12 months. As long as your child’s last well visit is within this time frame, just provide us with a copy of the sports form and the completed form will be available for pick-up or in your child’s portal within 72 hours. Some sports associations require that forms be completed based on a physical within the calendar year, and sometimes even more recently than that.  Because most insurance plans do not cover more than one well-visit per 12 month period, Weiss Pediatric Care offers a Sports Physical for a self-pay fee of $40. If your child’s sports form requires a well visit, please call as soon as possible to schedule a Sports Physical.
